General Notes: Based on definitions provided by HUD, "sheltered chronically homeless individuals" refers to people with a disability who are staying in emergency shelters, transitional housing programs, or safe havens and who have been continuously homeless for 1 year or more or have experienced at least four episodes of homelessness in the last 3 years where the combined length of time homeless in those occasions is at least 12 months. Individuals may be homeless as single adults, unaccompanied youth, or in multiple-adult or multiple-child households.
